THE PICTURE · 5–10 YEARS

What the future looks like

In five to ten years, brain–computer interfaces (BCIs) could be an approved, reimbursed treatment: people with paralysis or ALS typing, talking and moving robotic arms by thought, and some blind people getting a basic form of sight back through an implant in the visual cortex.

The long-term bet goes further. Elon Musk founded Neuralink to give humans a high-bandwidth link to AI, so that people can keep up with superintelligence rather than be left behind. Bionic limbs driven by the nervous system, and perhaps by robot hardware such as Tesla’s Optimus, would extend that from the brain to the body. None of the “superhuman” part exists yet. FACT CHECK

What works today vs. what is still aspirational

Works today: thought-controlled computers. Neuralink said on 28 January 2026 that 21 people were enrolled in its trials, up from 12 the previous September (Reuters, 28 Jan 2026). Independent trackers count at least 27 implanted participants by October 2026, in the US, UK, Canada and the UAE (Neura Pod, Neuralink by the numbers, Oct 2026 (independent tracker)). Participants with paralysis or ALS control cursors, phones and computers by thought.

Works today, in trials: other implants. Synchron’s Stentrode is placed through a blood vessel rather than open brain surgery; its US early-feasibility study, INTENT, is recruiting about 10 participants (ClinicalTrials.gov, Synchron INTENT study NCT07543367). In a 2024 NEJM study, a UC Davis speech BCI let a man with ALS speak through a computer at about 97.5% word accuracy. That was one participant (Card et al., NEJM 2024 (open access)) (UC Davis Health, Aug 2024).

Works today, at scale: hearing. Cochlear implants are the original mass-market neural interface. About 736,900 had been implanted worldwide by December 2019 (NIDCD (U.S. National Institutes of Health)), and more than 1 million by 2022 (NIDCD, Quick statistics about hearing).

Works today, in research: bionic legs. In a 2024 Nature Medicine study, seven people with below-knee amputations walked with a prosthesis driven by their own nervous system. Their top walking speed rose 41%, to roughly that of people without amputations (Song et al., Nature Medicine, Jul 2024) (MIT News, 1 Jul 2024).

Not yet: restored vision (Blindsight). The FDA gave Blindsight “breakthrough device” designation in September 2024 (Neuralink, Sep 2024) (Reuters, 17 Sep 2024). That speeds up review but is not approval: an independent review citing FDA figures counts 1,284 designations against 198 authorizations (Independent review of Blindsight claims citing FDA figures, 2026). In June 2025 Musk said first human vision implants would come “in the next six to 12 months” (MobiHealthNews, 23 Jun 2025 (Musk at Y Combinator)). As of October 2026, Neuralink had not announced a Blindsight trial (Neura Pod, Blindsight trial status, Oct 2026).

Aspirational: superhuman senses, Optimus bodies and AI symbiosis. Musk has described infrared and ultraviolet vision “like a superpower situation” (MobiHealthNews, 23 Jun 2025 (Musk at Y Combinator)), Neuralink paired with Optimus robot limbs (Business Insider, Sep 2023, on Musk’s post on X), and “symbiosis with artificial intelligence” (Axios, 17 Jul 2019). No Neuralink–Optimus limb has been shown working in a person, and no implant has made anyone smarter than normal. Treat these as goals, not products.

SIGNPOSTS

What we're watching

RISKS

What could go wrong

  • Brain surgery carries real medical risk, and long-term durability of implants is still being studied.
  • Timelines slip: the Blindsight implant Musk expected within a year in June 2025 had not happened by October 2026.
  • Neural data is the most personal data there is. Privacy, consent and who owns it are open questions.
  • Ethics of enhancement: if implants ever give an advantage, access and fairness become real problems.
  • Investors can’t buy Neuralink directly. It is a private company, so exposure through listed stocks is indirect at best.
